Berkeley College: Admissions Checklist

Here’s a list of the items you’ll need for your Admissions file:

Official high school transcript or GED.

Official college transcripts(s) including transcripts showing completion of an accredited Practical Nurse program.

Immunization records. If you are over 31 years of age, please provide a photo ID that shows your date of birth (DOB).

Health Education Systems Incorporated (HESI) score of 75 or higher. Bring photo ID when testing.

Copy of current, active practical nurse license.

Employer verification of having worked a minimum of 2,080 hours within the last two years as a Licensed Practical Nurse. Click to see an employer verification sample.

Two professional letters of reference, with at least one being from a current or recent employer/supervisor. Letters must be signed in ink.

Background check completed successfully.

Financial Aid

To apply for financial aid, you must submit the FAFSA (Free Application for Federal Student Aid).

Please note:

Students starting the winter 2026 term must file the 2025-2026 FAFSA using your 2023 taxes.
Students starting the spring 2026 term must file both 2025-2026 using your 2023 taxes and 2026-2027 FAFSAs using your 2024 taxes.
Students starting the fall 2026 term must file the 2026-2027 FAFSA using your 2024 taxes.

The College encourages all students to submit the most recent FAFSA available each year starting October 1st. Doing so ensures your financial aid can be determined in a timely manner.

Be sure to use Berkeley College’s school code New Jersey: 007502

ATTENTION: If the FAFSA determines that you are a Dependent student, your parent/contributor will need to create their own FSA ID and password.

After submitting the FAFSA, a confirmation number will be displayed for your records.
