Admissions File Requirements

Here’s a list of the items you’ll need for your Admissions file:

Official high school transcript or GED.

Official college transcripts(s) including transcripts showing completion of an accredited Practical Nurse program.

Immunization records. If you are over 31 years of age, please provide a photo ID that shows your date of birth (DOB).

Health Education Systems Incorporated (HESI) score of 75 or higher. Bring photo ID when testing.

Copy of current, active practical nurse license.

Employer verification of having worked a minimum of 2,080 hours within the last two years as a Licensed Practical Nurse. Click to see an employer verification sample.

Two professional letters of reference, with at least one being from a current or recent employer/supervisor. Letters must be signed in ink.

Background check completed successfully.

Financial Aid

Step-by-Step Application Guide

1. Create Your FSA ID

Go to StudentAid.gov to create your Federal Student Aid (FSA) ID and password.

2. Complete the FAFSA

Submit your FAFSA application.
Important Term Note: Students starting in Fall 2026 and Winter 2027 should complete the 2026–2027 FAFSA using 2024 tax information.

3. Add Berkeley College

Be sure to enter the correct school code for your primary campus location:

4. Invite Your Contributor (If Required)

If the FAFSA determines that you are a dependent student, your parent or contributor must create their own FSA ID and password to complete and sign their portion of the application.

5. New York State Aid (NY Residents Only)

After submitting the FAFSA, click the blue state aid link on the confirmation page to provide additional information required for NY State grants/aid.
