Reporting of Errors and Near Misses

While in the clinical setting, student and faculty responsibilities after an incident vary slightly depending upon whether the incident was a near-miss or an error. The primary responsibility of the student and instructor following a near-miss or error in the clinical setting is to take care of the patient’s immediate needs and notify the patient’s primary RN and health care provider.

After the patient’s needs have been met, the student and Berkeley College faculty will report the incident to the necessary individuals and complete the facility’s required internal incident documentation as well as notify the College’s Nursing Department Chair.

The clinical faculty member, in collaboration with the Department Chair, will create a remediation checklist to include activity(ies) that must be successfully completed in the on-campus laboratory prior to returning to the clinical site. Failure to complete the required remediation may result in course failure and/or program dismissal.

All documentation related to the clinical error(s) and/or near miss(es) will be kept in the student’s Image Now file.
