Clinical Clearance Requirements
In preparation for clinical rotations, please follow the guidelines outlined below. Students should also refer to the Clinical Clearance Canvas course for specific details on each of the items below and are encouraged to reach out to the Clinical Coordinator with specific questions and/or concerns.
Week 1
Due by Saturday 11:59 PM
- Government-issued photo identification (i.e., passport, driver's license, etc.)
- Proof of health insurance
- Proof of COVID vaccination
Week 7
Due by Saturday 11:59 PM
- Completed student and healthcare provider health forms
- Lab work, including applicable titers
Week 10
Due by Saturday 11:59 PM
- Basic Life Support BLS card (must be current for entire length of clinical rotation(s))
- Proof of flu vaccination (seasonal)
- Proof of FIT testing (details regarding testing will be provided)
Week TBD
Due by Saturday 11:59 PM
- Urine drug screening
- Background check (details regarding this will be provided)
Students who fail to submit health forms in their totality by the given due date will be withdrawn from the clinical course and a course designation of WF will be assigned. All students are encouraged to retain copies of all documentation presented to the Clinical Coordinator. The Allied Health Surgical Department reserves the right to make administrative changes to clinical placements as necessary.
